ElderGAP® 

ElderGAP® is a service that provides a geriatric case manager intervention that assists seniors and their families obtain necessary on-going care.

Often these are seniors not being served by other programs and thus do not have a case manager. ElderGAP® involves a professional assessment and follow up actions to best protect the senior.
