CARE TRANSITONS TRANSITION COACH, HEALING@HOME PROGRAM – English/Spanish bilingual
Under the supervision of the Care Transitions Project Director, the Transition Coach is the central coordinating point for the Healing@Home Program responsible for identifying, qualifying, enrolling, assessing and implementing individual care transition plans for all Medicare beneficiaries who meet program eligibility requirements. Responsible for the overall coordination and delivery of all home and community-based services and post-discharge services identified on the patient’s discharge plan, this position ensures that the participating Medicare beneficiary receives all Healing@Home Services following program protocols and timelines that reflect the implementation of an “intensified Coleman care transition model.”
