HIV Care DirectionsCare Directions LogoHIV Care Directions® (HIVCD) is a nationally accredited, professional case management program that has provided medical and support case management services to HIV positive people 18 years and older since 1992.
HIVCD Case Managers conduct bio-psycho-social assessments, develop individualized care plans, and provide linkages to services to meet each person’s identified needs.

Those services include access to and maintenance of primary medical care and medications, transportation for HIV care related services, and a variety of additional medical and support services for their clients.

Case Managers work extensively with medical providers, AIDS service organizations, government entities, and every community resource to meet those needs and accomplish an individual’s healthcare goals.
